Implemented standalone User-level function "Register AutoFocus3D slice-by-slice"

AutoFocus4D dialog box:  encapsulated the dialog header into the code file and exposed the global function for calling the dialog.

AutoFocus4D registration:  Reduced SliceMode option to  checkbox Mode2D. Moved this option to the MEASURE panel.
